EXPERIMENT SAFETY ASSESSMENT FORM (ESAF)
The non-X-ray ESAF is required for all Experimental Support Services (ESS) laboratory work and must be fully approved before any activities begin.
What to Include in the ESAF
 
Description of Activities

Provide a clear description of all planned laboratory activities, such as:

  • Sample handling (mixing, grinding, loading, heating, cooling, applying voltage, etc.)
  •  
Materials & Equipment
  • List all materials (chemicals, gases, cryogens, etc.) along with their known hazards.
  • List all equipment (furnaces, power supplies, gloveboxes, and any specialized instruments).
